Our adult day program is a specialized program for high functioning seniors that desire social interaction, good food, health monitoring, and an overall sense of belonging to a community. It is a perfect blend of personalized interactions combined with a clinical support team around the clock to ensure the overall well-being of those involved with the program.
What makes us special is our focus on the whole person and balance in understanding the social, physical, intellectual, and spiritual needs of each participant. Each life represents a body of experience and knowledge that can be shared to benefit all participants in the social club.

Virtare Services (call to learn more) For those that have insurance that qualifies*, additional 24/7 support services are offered with remote monitoring and care coordination through Virtare Health. *Insurance that qualifies for Virtare Health services are traditional Medicare and most Medicare PPO plans.

Do you offer Kosher or vegetarian meals?Yes. We should offer both, but would specify with each participants initial intake care plan.

Benefits of Adult Day CareThe main benefit of adult day care is delaying or preventing the move to a long-term residential care facility so older adults can live at home or in the community as long as possible. Benefits of adult day care can also include: A safe, secure setting for adults with Alzheimer’s disease, dementia or mobility issues who could wander from home, fall or injure themselves if left alone Social activity and companionship, which can bolster mental and physical health A reduced risk of falls with exercises that strengthen muscles and improve balance Enhanced sleep quality as a result of consistent exercise and social activities Reduced caregiver stress, burnout, anxiety and depression

How do people qualify for Senior Social Club for Senior Care?Each participant will be given a private and confidental evaluation. Family members and loved ones are welcome to join and we will be in communication with primary physician if so elected.

Who Should Consider Adult Day Care?Adult day care is ideal for older adults with cognitive impairments, such as dementia, or for homebound seniors who are isolated and need social interaction and some physical activity. Generally, participants in adult day care programs are incapable of managing and structuring their daily activities on their own due to cognitive and/or mobility issues. Older adult participants aren’t the only ones who can benefit from adult day care programs. Family caregivers who must work during the day or need a break from stressful, ongoing caregiving obligations can get much-needed time to recharge so they can continue to offer compassionate care to a loved one.

What Is Adult Day Care?Adult day care, often referred to as adult day services, provide a supervised community setting for older adults with dementia, Alzheimer’s disease and other health conditions that prevent them from being home alone safely during the day. Programs usually include activities for mental and physical stimulation, such as exercises, games, art and more. “The two primary purposes of adult day care centers are to provide adults, particularly frail elders, with medical, health or functional support and to allow caregivers to work at a regular job while receiving care for their family member,” says Elise Eifert, Ph.D., a gerontologist, assistant professor and gerontology graduate coordinator at the University of North Carolina in Greensboro.

What is Senior Social Club for Senior Care?Senior Social Club is a licensed community based adult day program that provides daily services to the elderly and adults. Our Programs are individualized for each participant based on their needs and plan of care. Independence, self expression, and being part of a group helps adults, who may be isolated by illness, disability or loss, maintain a positive orientation on life.

VIRTARE SERVICES
"Virtare Health", which is a Healthcare management group that provides Enhanced Care Management, which is covered by most insurances". Virtare is virtual care. We support Senior Social Club to drive outcomes to increase patient engagement and adherence with our virtual clinical teams — chronic care management (CCM), remote patient monitoring (RPM), transitional care management (TCM), remote therapeutic monitoring (RTM), enhanced care management (ECM), behavioral health integration (BHI), and Teleplacement. Our panoptic healthcare teams, technology, and ecosystems are a safety net to enhance a patient’s care circle and preventative care to improve outcomes and aging in place.
WELLNESS ASSESSMENTS
Our team members perform a thorough evaluation of each new participant and propose support services based on their particular needs, goals and desired outcomes. We encourage family members, caregivers, and/or individuals of the participant’s to fully participate in the service planning process. This recurring process is part of our ongoing commitment to each and every participant.
CARETAKERS
Our professional caretakers organize care, manages medications, observes and monitors participants’ health status and any changes in their condition. In addition, our caretakers work closely with the participant, family and/or caregiver, and primary physician to coordinate and provide total care. Our panoptic healthcare teams, technology, and ecosystems are a safety net to enhance a patient’s care circle and preventative care to improve outcomes and aging in place.
PERSONAL CARE ASSISTANCE
Our team members provide constant care and supervision to ensure our participants’ safety and protection. Assistance is provided with walking, dressing, grooming, eating and all personal hygiene needs, including toileting and continence care. Cleanliness and safety are essential to our quality of care.
SOCIAL SERVICES
Social Service Referrals to Community-based Services.
Our team of social workers engage each participant upon admission to evaluate their social and psychological needs. Social workers act as a liaison with the participant’s family, home and other community agencies. In addition, the social worker provides a home safety assessment and meets with each participant on a frequent basis. Our Social Workers provide a personal touch to each of our participants to keep them connected within the community and make them feel like family.
THERAPEUTIC ACTIVITIES
Group and individual activities are offered to boost the social, physical and cognitive functioning of the participant. Person-aligned activities are designed to be fun, stimulating, educational and entertaining. Aided participation is provided to those whose frailty or level of cognitive functioning inhibits them from active involvement.
PSYCHIATRIC / PSYCHOLOGICAL SERVICES
Services are provided for the treatment, restoration and stabilization of a diagnosed mental disorder by a licensed mental health professional on staff. Individual and group counseling is provided which seeks to help individuals improve their health and wellness, live a self-directed life, and strive to reach their full potential.
EXERCISE, CULTURAL and RECREATIONAL ACTIVITIES
All activities have been designed to encourage a balanced healthy approach to daily living, and sometimes… just for fun! Please see our calendar for daily activities.
TRANSPORTATION SERVICES
Assistance with transportation coordination is provided. Contact our team directly to see how we can help our participants and their families. Safety and reliability are of the utmost importance.
DIETARY SERVICES
"We are what we eat! Our focus is to keep our participants healthy, inside and out. Each participants dietary needs will be assessed to ensuring balanced and safe meals that accommodate and meet the nutritional needs of the individual. Mealtimes are also a great source of social connections – a time to socialize with other participants. We serve breakfast, hot lunch and snacks.
